why do you try to mimic the Outlook/Exchange situation? Is there any special reason to do so?
With using Notes / Domino, your basic setup misses A LOT of Notes / Domino features, which might be very helpful for your company. Among them are
- mail journaling
- virus checking (if you had an anti-virus tool at the server)
- mail tracking
- server-based MIME and address conversion
- more administrative control of what's going on
- much easier configuration
and so on.
I fear that many of the SMPT problems you have posted are directly related to this infrastructure setup, which does use the abilities of Lotus Domino very efficiently.
If I were in your situation, my basic scenario on outgoing mails would be:
- my clients would send ALL mail to their Domino home server
- the Domino server would then send these mails either directly to the internet OR through an SMTP forwarding host (located possibly at your ISP).
However, I'd prefer mail being sent directly to the internet from the Domino server. If your firewall supports NAT, you don't need to physically assign a "legal" IP address to your server, you just need to have it (so the firewall can assign it).

NB: I also have massive security concers on your POP3 incoming mail setup ... that also is not the most efficient use of Domino mail
